Comprehensive new patient intake including medical and psychiatric history, diagnostic evaluation, and treatment planning.

Accepting patients 18 years and older.
Billing & Cancellation Policies
Payment is due at the time of service. I accept credit/debit cards and HSA/FSA cards.
Please provide at least 24 hours’ notice to cancel or reschedule your appointment.
No-shows and Cancellations with less than 24 hours’ notice will be a $75 fee
Insurance coverage varies by plan; I can provide a superbill for potential reimbursement if I am out-of-network. Please confirm benefits with your insurance provider.
